What is Biometric Screening?

The biometric screening consists of an overnight fast followed by a finger stick blood draw, as well as basic anthropometric measurements such as height, weight, body fat %, waist circumference, waist/hip ratio, etc.

This process should take around 30 minutes to complete.

*NOTE: This is NOT a drug test.

Due Date: Biometrics must be completed and turned into iaWellness by September 30th, 2011.

How do I schedule my Biometric Screening?

Step 1: Select "Login/Register" at the top right of your screen

Step 2: Log in with your Wellness ID and password

Step 3: Select "Schedule Biometric Screening" on your personal timeline

Step 4: Follow the instructions to schedule an appointment

On-site Biometric Screening Dates:

The My Wayland Wellness 2011-2012 Wellness Screening will start in August.

Biometric Screening Dates

Plainview: August 22nd and 23rd

Lubbock: August 26th

Wichita Falls: August 31st

San Antonio: September 9th

For 2011-2012 Biometric Screenings, the print the PDF documents below, which contain instructions for you to complete your wellness screening and the Biometric Results Form. Your Biometric Results Form needs to be completed and signed by your personal care physician (PCP).

After completing all 4 steps on your Biometric Results Form please submit your results and form to iaWellness via mail, email, or fax.

NEW HIRE DEADLINE

New Hires are not required to get screened, but can rewarded 75 points for getting screened. Screening results must be sent to iaWellness via mail, email, or fax.
